She still works as a doctor -она до сих пор работает врачем.Она делает это день ото дня (это факт,что она работает врачем) и поэтому это предложение в пресент симпле (она делает это регулярно:работает врачем)

Если вы говорите has been working то должны сказать период того сколько она уже работает (говорите или само время работы,либо когда это началось)
